Certification card (PA EMT, Paramedic)
Employment ID badge from EMS agency
Letter from EMS agency verifying service
Retirement or resignation letter (for former personnel)

Letter of verification from the Fire Chief on official department letterhead
Membership card (must be verified by the department)
Firefighter I or higher training certificate
